Case Planner

The Case Planner provides in-home counseling and support to families to help them develop skills and competencies, enabling them to keep their children at home and out of placement. This role involves maintaining a caseload of 12-14 families, conducting comprehensive assessments, and delivering change-focused interventions over a 6 to 12-month period. The position requires flexibility to work late nights and meet families in various locations to address clinical and concrete needs.
