Case Worker Outreach

Coordinates exchange of clinical and medical information between admitting psychiatric or correctional facilities and Brown University Health's Gateway treatment program to support transitions to community-based care. Provides short-term casework and delivers substance abuse treatment in alignment with ICRC core functions while facilitating discharge planning and interagency coordination.
