Caseworker

The Caseworker provides ongoing case management and advocacy services for youth within the Close to Home Programs division, fostering relationships with families, schools, courts, and other service providers. The role involves coordinating discharge planning, maintaining required documentation, and advocating for educational, vocational, and supportive services while ensuring culturally responsive, child‑centered care.
