CDCA Case Manager

The CDCA Case Manager provides comprehensive, recovery-oriented case management services to support sustained recovery and overall wellbeing for patients at Basecamp Recovery Center. This role involves developing individualized case management plans, connecting patients to community resources, and serving as an advocate to ensure access to necessary services and supports. The position operates within the clinical department under the direction of the Case Manager Lead.
