About this role
The Care Extender/Peer Support Specialist is a field-based role within the Children and Families Specialty Plan (CFSP) partnering with North Carolina DHHS to support Medicaid-enrolled children, youth, young adults, and families involved with the child welfare system. The position operates across Cherokee, Clay, and Graham counties, using professional expertise and lived experience to connect members and caregivers with resources and supports and to facilitate integrated, coordinated care.
