CFSP Care Extender/Peer Support (Cherokee/Clay/Graham)

The Care Extender/Peer Support Specialist is a field-based role supporting Medicaid-enrolled children, youth, young adults, and families served by the child welfare system under North Carolina’s Children and Families Specialty Plan (CFSP). The position partners with community stakeholders and DHHS to help members access integrated physical, behavioral, and support services across Cherokee, Clay, and Graham counties. The role emphasizes use of professional expertise and lived experience to connect members and caregivers with resources and navigation assistance. North Carolina residency is required and the position operates primarily in the field with virtual work from home when not onsite.
