About this role
The CFSP Young Adult in Transition Coordinator supports implementation of the Children and Families Specialty Plan (CFSP) to help Medicaid-enrolled young adults (ages 18–26) receive coordinated, integrated care. This field-based role operates within North Carolina DSS Region 1 and works closely with the System of Care Director to advance statewide system of care efforts for child welfare populations. The position represents young adult members in community forums and supports local-level operationalization of CFSP policies.
