The Care Coordinator/Family Resource Partner partners with a Mental Health and Developmental Clinician to support families enrolled in the Child First early childhood home-visiting model. The role connects children and caregivers to community-based services, supports caregiver executive functioning, and promotes healthy development for infants and young children. This position is field-based, requiring engagement with families in their homes and participation in reflective clinical supervision and training.
