Choice Case Management and Relocation Team Supervisor

This role directs and manages the Choice Case Management and Relocation Team while maintaining a personal caseload of families formerly residing at Chouteau Courts. The position involves assessing family needs, developing individualized care plans, and coordinating services with community partners and the United Way to promote self-sufficiency and successful relocation. The incumbent supervises team activities, tracks program metrics, and ensures compliance with HUD regulations and grant requirements.
