The Clinical Care Partner ensures clients receive person-centred, high-quality care by overseeing and coordinating complex clinical and care needs through assessment, care planning, and service coordination. The role focuses on achieving optimal care outcomes for clients funded through Aged Care Services by managing budgets and facilitating multidisciplinary care activities. This position involves significant travel within the community to engage directly with clients and monitor their changing needs.
