BlueCare
The Clinical Care Partner ensures clients receive person-centred, high-quality care by overseeing and coordinating complex clinical and care needs through assessment, care planning, and service coordination. The role focuses on achieving optimal care outcomes for clients funded through Aged Care Services by developing and reviewing complex care plans in collaboration with clients and multidisciplinary teams. This position involves managing care budgets, liaising with clinical teams to monitor changing client needs, and assisting clients in navigating the care system to access necessary resources.
