Clinical Case Manager

The Clinical Case Manager supports the treatment process by coordinating care for clients and families, strengthening communication across the treatment team, and ensuring each client’s treatment journey is organized and aligned with clinical goals. This role involves collaborating with therapists, psychiatrists, and external providers to deliver comprehensive, coordinated care that supports clinical progress and long-term stability. The position reports to the Program Supervisor and plays a vital role in maintaining program integrity and client well-being.
