Clinical Case Manager

The Clinical Case Manager provides community-based interventions to stabilize and support high-need individuals on the Intensive Case Management team in Multnomah County. This role involves implementing, monitoring, and documenting services through a trauma-informed and DEI-focused lens, while collaborating with Mental Health Clinicians to increase contact and crisis response. The position requires significant community outreach, coordination with providers, and advocacy to promote participant wellness and housing stability.
