Clinical Coordinator Hospice

As a Clinical Coordinator at St. Luke's, you will maintain clinical practice standards, guide the interdisciplinary home health team, and liaise with Patient Care Managers to ensure optimal patient care and discharge planning. You will assist in developing staffing adjustments, support staff education and competency training, and actively participate in program performance improvement initiatives within the nursing network.
