About this role

The Clinical Lead joins Strive Health’s interdisciplinary team to provide longitudinal, high‑touch care for patients with chronic heart failure, chronic kidney disease, and related conditions. Working remotely and on‑site as needed, the role leverages advanced analytics, telehealth, and home visits to keep patients out of the hospital and slow disease progression.
