About this role

The Clinical Nurse Consultant provides expert palliative care mentorship, education, and direct patient support across community settings, including home, long‑term care, and retirement homes. Working as a lead educator and on‑call consultant, the CNC collaborates with interdisciplinary teams to improve hospice palliative care delivery and patient outcomes in the Ontario Health Central Region.
