Clinical Nurse Coordinator

The Clinical Nurse Coordinator collaborates with internal partners and external agencies to ensure the safe, effective, and timely admission of patients to NuFactor services. This role coordinates patient care activities and nursing schedules to promote continuity of care while serving as a clinical and operational resource for subcontracted and internal nursing staff. The position involves facilitating patient transitions to home care, supporting home infusion nursing services, and maintaining compliance with clinical documentation standards.
