About this role
The Transitions of Care/Boarders Clinical Pharmacist at Mass General Brigham provides clinical pharmacy services focused on optimizing medication therapy and care transitions for boarder patients located in the Emergency Department. The role supports interdisciplinary collaboration to ensure safe, effective, and prompt medication management and supervises pharmacy technicians and learners during medication histories. This onsite position is based at the Main Campus and follows an 8-on/6-off rotating schedule.
