The Clinical Resource Coordinator is a registered nurse who coordinates clinical management and delivery of wound care services within an outpatient physician practice. The role acts as a team lead and central point of contact for patients, families, providers, and multidisciplinary teams to ensure continuity of care and development of individualized care plans. This is a per-diem, onsite position based at Massena Hospital.
