Clinical Social Worker

This role involves performing psychosocial assessments and providing psychotherapy to patients and families coping with medical issues. The position requires collaborating with a multi-disciplinary team to establish psychosocial determinants for safe discharges and serving as a consultant regarding the psychosocial components of illness. The incumbent also facilitates staff emotional debriefing, provides crisis intervention, and coordinates post-acute care referrals.
