Clinical Social Worker

The Clinical Social Worker provides psychosocial assessment, counseling, care coordination, and advocacy for residents and their families across a full continuum of care, including Independent Living, Assisted Living, Memory Care, and hospice. This role supports emotional well-being and collaborates with an interdisciplinary team to ensure person-centered care. The position involves building relationships with residents, conducting assessments, and offering education and resources on topics such as dementia and end-of-life care.
