Clinical Social Worker

This role provides biopsychosocial assessments, crisis intervention, and short-term counseling for patients and their families across all ages and care settings. The position involves facilitating support groups, consulting on psychosocial care as part of an interdisciplinary team, and assisting with discharge planning and transitions of care. The Clinical Social Worker also serves as a liaison to community programs and provides education to staff and hospital departments.
