Clinical Social Worker

This role provides biopsychosocial assessment, crisis intervention, and short-term counseling for patients and their families across all ages and care settings. The position involves facilitating support groups, consulting on psychosocial aspects of care, and collaborating with interdisciplinary teams on discharge planning and transitions of care. The Clinical Social Worker also assists with goals of care conversations and provides education to staff and hospital departments.
