About this role

The Clinical Team Lead supports the operational and clinical division of a Medicare-certified home health agency, reporting to the Clinical Director/Clinical Manager. The role ensures continuity of patient care across nursing, home health aide, social work, and therapy services and maintains medical records and individualized plans of care. The position also participates in quality and performance improvement activities and may assume the Clinical Director/Manager role in their absence.
