This role focuses on helping patients transition from inpatient settings back home or to post-acute care while developing and monitoring care plans for patient self-management. The position utilizes motivational interviewing techniques to enhance outcomes and addresses social determinants of health by referring patients to community resources and social worker support. Responsibilities also include accurate documentation in medical records and CareTrac, as well as assisting providers with closing care gaps through patient outreach.
