Under the direction of the Director of Community Services, this role manages all aspects of the CLU program, including staff supervision, grant and contract management, budget oversight, and patient advocacy. The position serves as a liaison between the CLU program and other AHS departments, providing community outreach, education, and care coordination to improve patient health and community engagement. It focuses on program operations and health improvement initiatives rather than technical or IT project management.

Responsibilities

Provide patient and community education, psychosocial service referral, and linkage to care
Supervise CLU program staff and act as liaison between program and other departments
