About this role
The Behavioral Health Care Manager provides comprehensive, person-centered care management as part of MAHEC’s Population Health and primary care teams. The role coordinates mental and physical health services for an assigned caseload, supports outreach and transitions of care, and systematically tracks patient outcomes. The position works with clinical providers, psychiatric consultants, community partners, and care navigation staff to optimize care delivery and close care gaps.

Qualifications
- Bachelor’s Degree in Human Services

- Licensed Clinical Social Worker
- Registered Nurse
- Qualified Professional per 10A-NCAC 27G.0104
- Valid North Carolina Driver’s License
- Flu Vaccination or Approved Exemption
