Community and Street Outreach

This position is responsible for community and street outreach, serving as an active part of the San Joaquin Valley Veterans outreach team. The role involves providing screening, assessment, and brief interventions to homeless individuals and those at risk, while coordinating with community resources to promote their wellbeing. The position requires participating in case staffing, team meetings, and maintaining a working knowledge of available community resources.
