About this role

The Community-Based Nurse Practitioner provides trauma-informed, culturally safe primary care to Indigenous patients across multiple settings, including clinics, partner sites, mobile clinics, and home visits throughout the Simcoe Muskoka region. Working within an interprofessional team, the NP collaborates with nurses, physicians, traditional healers, and other providers to deliver comprehensive, wholistic health services while contributing to service planning and coordination.

Responsibilities

Provide primary care services to Indigenous patients across multiple settings
Collaborate with interprofessional team to plan and coordinate community health services
Travel regionally using mobile clinic to deliver care where patients are

Qualifications

Baccalaureate Degree in NursingCertificate as a Primary Health Care Nurse PractitionerMember in good standing with the College of Nurses of Ontario (RN(EC))Valid Class G Ontario Driver’s LicenseCurrent CPR and AED Certification
