Community Guide

The Community Guide at Reema Health builds trusting, community-based relationships with members who have complex health and social needs to improve their health and wellbeing. The role focuses on outreach, enrolling members into Reema’s care model, supporting members through their care journey, and coordinating connections to healthcare and community resources. The position requires significant fieldwork and use of Reema’s technology platform to document interactions and manage caseloads. The Guide reports to the Care Team Manager and works in a fast-paced, evolving startup environment.
