Community Health Director

The Community Health Director leads the YMCA's efforts to improve community health outcomes by developing and growing evidence-informed chronic disease prevention and management initiatives. This role oversees wellness programs, staff, and facilities while building partnerships with healthcare providers and community organizations to expand access to integrated health services. The position also involves advancing secure referral and data-exchange processes to support coordinated programming with medical partners.
