The Community Health Navigator supports Hartford residents in accessing and staying engaged with medical, behavioral health, and social services. This role involves community outreach, patient navigation, whole-person health education, and barrier-reduction strategies to promote whole-person health. The position conducts outreach to individuals not currently connected to care and reengages existing patients using SDOH screening data to develop individualized navigation plans.
InterCommunity Health Care is a FQHC (Federally Qualified Health Center) that provides primary care, behavioral health care, and addiction recovery services in the Greater Hartford area. Our centers in Hartford, East Hartford, and South Windsor, Connecticut provide same-day care to any community member who needs it, regardless of life situation or ability to pay.
