About this role
The PACE Community Health Nurse (PCHN) provides skilled nursing assessments and routine clinical visits to PACE members across community and institutional settings, operating as a core interdisciplinary team member under a Nurse Manager. The role focuses on maintaining regulatory compliance, completing UAS-NY assessments, and supporting member eligibility and continuity of care within the PACE model. The position requires travel to member homes, PACE centers, hospitals, and nursing facilities as needed.
