Community Health Worker (temporary)

Mosaic is seeking a temporary Community Health Worker to serve as a bridge between patients and the care and social services they need. This role involves working alongside a clinical team in clinic and community settings to help patients navigate systems, remove barriers, and achieve better health outcomes. The position requires THW certification or willingness to obtain it upon hire.
