The Community Health Worker will join Amae Health’s Upper East Side clinic as a full-time, onsite member of an interdisciplinary team providing outpatient care for individuals with severe mental illness. Reporting to the Practice Operations Manager, the role supports continuity of care by connecting patients to community resources and coordinating across clinical and social services. This position emphasizes patient advocacy and engagement to improve outcomes and reduce repeat hospitalizations.
