The Community Health Worker (CHW) connects underserved and vulnerable populations to health and social service systems, building trust and supporting patients across their care journey. The role supports clinical teams and management by developing community outreach programs and assisting patients with access to resources, paperwork, and referrals. The CHW reports to the Clinical Leader and may work across offices, clinics, and patient homes.
