The Community Health Worker (CHW) connects underserved and vulnerable populations to health and social service systems, supporting patient access to resources and removing barriers to care. The role supports medical providers and leadership by contributing to community outreach programs, tracking referrals and outcomes, and maintaining resource inventories across service areas. This position reports to the Clinical Leader and may require in-person work in offices, clinics, and patient homes as well as remote duties.
