The Community Health Worker is a full-time onsite role based at Amae Health’s Upper East Side clinic supporting individuals with severe mental illness. The role partners with an interdisciplinary team and reports to the Practice Operations Manager to improve continuity of care, connect patients to community resources, and support transitions from inpatient settings. This position represents Amae Health in hospital, community, and home settings to engage and educate potential and current members.
