The Community Health Worker supports CalAIM clients by providing care coordination with a preventative health focus to help aging adults and adults with disabilities maintain independence and age in place. The role connects clients to healthcare and social services, addressing barriers to access for those with complex biopsychosocial needs. It operates within interdisciplinary teams to support client enrollment in waiver programs and continuity of community-based care.
