About this role

The Community Health Workers (CHW) at AltaMed are trusted community members who assist vulnerable patients with chronic conditions, preventive care, and social determinants of health. They provide essential services including health care navigation, referrals, trauma-informed care, and cultural brokering to improve access to quality medical services. CHWs receive training in Epic and Essette systems along with specialized modules on harm reduction, advocacy, and basic case management.

Responsibilities

Support patients with healthcare navigation and specialty referrals
Provide trauma-informed care and culturally appropriate health information
Assist with insurance enrollment and social service applications
Act as a cultural broker for vulnerable community members
Facilitate preventative health screenings and chronic disease management

Skills

Qualifications

High School Diploma or GEDBLS Certification (AHA or Red Cross)2 years of experience in healthcare or community outreachBilingual Proficiency (English/Spanish/Mandarin/Cantonese)
