Community Health Worker

The Community Health Worker supports members with complex medical and social needs by managing their health and wellbeing through education, peer support, and behavioral change motivation. This role involves case management, outreach, and connecting patients to local services, with a focus on serving unhoused communities and other CalAIM populations in Long Beach. The position requires significant field work and travel to engage with eligible patients and enroll them in the Akido ECM Program.
