Community Health Worker

As a Community Health Worker, you will help individuals and families overcome barriers to better health by connecting them with healthcare, food, housing, transportation, insurance, and other community resources. The role involves conducting outreach, managing a caseload, supporting food-access and chronic-disease prevention programs, and helping community members take meaningful steps toward their health and wellness goals.
