Community Health Worker

This role serves as a local member advocate and resource, supporting community-based member advocacy activities and assisting vulnerable members in managing their health care needs. The position involves collaborating with health care services teams to provide non-clinical paraprofessional duties in the field, such as meeting with members in their homes or nursing facilities. The advocate helps members navigate health plan benefits, access social services, and maintain Medicaid eligibility while conducting outreach to locate and support disconnected members.
