Community Inclusion Worker

The Community Inclusion Worker supports individuals experiencing chronic homelessness through the Journey to Social Inclusion (J2SI) program, helping them build community connections, pursue education and employment opportunities, and achieve personal goals. This outreach-based role involves working closely with participants to identify barriers, develop skills, and access resources that foster independence and social inclusion. The position requires a trauma-informed, person-centred approach and collaboration with case managers and community partners.
