Legacy Hospice
The Community Liaison will serve as the face of Legacy Hospice in the Charlottesville area, building trust-based relationships with physicians, referral sources, and healthcare partners to increase awareness and preference for hospice services. The role focuses on outreach, advocacy, and supporting patients and families during critical moments while driving census growth through referral development.
Providing compassionate hospice care, education, and support for all our patients and their loved ones so they are as comfortable as possible.
