Community Liaison

The Community Liaison builds and maintains relationships with community referral sources, healthcare professionals, and senior-focused organizations to promote the PACE Georgia program and support enrollment growth. This role increases program visibility through targeted outreach, collaborates with operations to align community engagement with organizational goals, and provides information to prospective participants and families. The position ensures timely documentation of referral activity in the CRM while adjusting outreach strategies to meet community needs.
