Community Liaison

The Community Liaison at Corus helps patients and families access compassionate end-of-life support or skilled rehab by connecting physicians, facilities, and community partners with the necessary education and guidance. This role involves building trust-centered relationships with referral sources, facilitating the referral and admissions process, and expanding awareness of Corus Hospice through outreach. The position serves as a responsive partner to families and providers, ensuring consistent and transparent communication aligned with CareM values.
