Community Liaison

The Community Liaison is responsible for marketing, education, and outreach to ensure timely access to Hospice, Palliative Care, and Transitional Care services while increasing community awareness of HSCC's offerings. This role involves collaborating with internal teams and external partners to promote optimal care, present educational programs, and maintain strong relationships with skilled nursing facilities and physicians. The position requires expertise in serving patients and families facing life-limiting illnesses, along with managing patient admissions and regulatory compliance.
