Community Navigator

The Community Navigator bridges the gap between community members and local resources through virtual and in-person interactions, supporting individuals in navigating local programs and accessing assistance. This full-time role involves conducting outreach, providing education on health topics, and assisting clients with navigating MiBridges and inbound calls. The position requires extensive local travel and collaboration with community organizations to ensure effective resource referral and care coordination.
