About this role

The Community Outreach Specialist at Vynca, also referred to as the Community Liaison, is responsible for developing and nurturing partnerships within the community to enhance patient engagement and drive referrals into Vynca’s Enhanced Care Management programs. This role involves executing strategic outreach plans, understanding local health market dynamics, and measuring the success of partnerships focused on social determinants of health. The position requires fieldwork throughout Orange County, necessitating travel and the ability to work independently, while engaging with diverse community stakeholders to support and enrich care initiatives for underserved populations.
